Hi, I'm a clinical coach in Oncology dept in Auckland.
We are following ADAC modules for our chemotherapy here in Auckland DHB.
I would like to get some ideas and ask how other DHBs and Australian health professionals are doing regarding safe IV access site for chemotherapy.

We recommend nurses to access forearm or hands for IV access due to serious potential extravasation risks especially with vesicants.
But when it comes to have a patient with really difficult IV access, is it okay to use Anticubital Fossa for IV access?
I heard the other countries using ACF IV access for chemo/immunotherapy.

If ACF is safe to use for vesicants, what kind of instructions that we need to follow? eg. informing pt about risk of extravasation? ensure pt keep their arm straight while we are giving chemo?

How do you guys practice in other hospitals?
