The ‘toolkit’ is a risk assessment tool that was originally developed by the United Kingdom Oncological Nursing Society (UKONS). The toolkit is designed to assist staff to identify and prioritise the presenting problems of adult cancer patients contacting 24-hour advice lines.
The Toolkit uses a Red, Amber and Green (RAG) scoring system which can be used to standardly assess, triage and direct patients to appropriate care.
Recognising the value of the toolkit in the Australian health care setting, an Australian working group was formed. The group shares involvement from 4 states & ACT, 23 hospitals and consisting of medical oncologist, senior nursing clinicians and pharmacy leads. This led to an Australian version of the toolkit being developed and agreed upon.
